Anybody ever create an Internet Explorer bookmark icon (favicon.ico) in
    Photoshop?

How is an icon created, and what format (GIF, BMP, etc) is used?

    Thanks!

You can use my ICO file format plugin to do this; download from
    http://www.telegraphics.com.au/sw/#icoformat
Create your image (RGB or Indexed Mode) and simply Save As
"favicon.ico" in the root directory of your web site. Add the relevant
LINK tag to your home page's HTML HEAD section (see enclosed README).
The Windows .ICO format is a specific format which can store "suites"
of related icons in different sizes and pixel layouts. Contrary to a
widespread misunderstanding it is NOT exactly the same as BMP.
